OSTEOPLASTIC MODELING OF ACETABULUM DURING ENDOPROSTHETICS OF PATIENTS WITH SEVERE DYSPLASIA

Aim. To analyze the results of hip replacement with the use of bone grafting of the acetabulum roof in patients with dysplastic coxarthrosis.

Material and Methods. Bone grafting of the acetabulum was performed in 37 cases out of 290 operations of primary total hip replacement. For plastic surgery, autofilament tissue of the resected femoral head and bone chips obtained from the processing of acetabulum were used. All patients had dysplastic coxarthrosis according to the classification of Crowe (I-IV degree). The average age of the patients was 29±4,68 years.

Assessment of the severity of pain was carried out on a visual analogue scale (VAS), quality of life analysis – on a standardized scale SF 36.

According to the x-ray data, the location and stability of the endoprosthesis components were evaluated after 3, 6, and 12 months.

The functioning of the hip joint in the pre- and postoperative periods was determined according to the Harris scale..

Results. According to VAS data, the intensity of pain before surgery was 8,9±0,5, and after a year 2,4±0,4, which indicates about pain reducing. The indicators of quality of life in the postoperative period increased 5 times. Long-term functional results on the Harris scale confirm the improvement of all parameters of joint functioning: before the operation 28,1±6,15, in the postoperative period after 12 months – 85,3±6,4.
